Application Documentation Requirements for new students:
Kindergarten:

A recommendation form from the student's current teacher must be submitted as part of the application.
Grades 1-8:

All students must submit a Transcript Request Release Form to your child’s current school giving permission to forward records to CDS.

For Grades 1-5, one recommendation form from the student's current classroom teacher must be submitted.

For students applying to Middle School, two recommendation forms (Language Arts and Math) are required.
